This is to inform the entire students of the Federal Polytechnic Idah that the institution authorities has published the list of required documents for the las documentation for the National Diploma (ND) and Higher National Diploma (HND) students.

Federal Polytechnic Idah ND Documentation Requirements

Below is the list of required documents for ND students:

1. Original Admission/Application form with PIN
2. Photocopy of Admission Letter & Acceptance of Offer of Admission
3. Original Undertaking Form
4. Certificate or Verified Statement of Result for O’Level
5. Original JAMB Result with Passport
6. Photocopy of Online JAMB Admission Letter – Institution’s Copy
7. Original Course Form
8. Photocopy of Screening Teller
9. Original Clearance from the HOD
10. Original Medical Certificate
11. Photocopy of Certificate of State of Origin
12. Photocopy of Birth Certificate or Declaration of Age
13. Two copies of School Fees Breakdown & Acknowledgement Slip
14. Photocopies of RRR & Acknowledgement Slips for All Payments
15. Photocopy of Oath Form
16. Two Current Passport Photographs

For HND students, the following documents are required:

1. Original Admission/Application form with PIN
2. Photocopy of Admission Letter & Acceptance of Offer of Admission
3. Original Undertaking Form
4. Photocopy of ND result/O’level Certificate
5. Original Course Form
6. Photocopy of Screening Teller
7. Original Clearance from the HOD
8. Original Medical Certificate
9. Photocopy of Certificate of State of Origin
10. Photocopy of Birth Certificate or Declaration of Age
11. Academic Transcripts
12. Original IT Letter, duly signed & stamped
13. Photocopy of School Fees Breakdown
14. RRR & Acknowledgement slips for all payment
15. Photocopy of Oath Form
16. JAMB Regularization Slip/Photocopy of JAMB Admission Letter (Institution copy)
17. Two Current Passport Photographs
